We collect and store personal information including your name, address, telephone number, date of birth, email addresses, past medical history  GP details and next of kin during our initial assessment and by those who communicate with us directly via email. 


What is my personal data used for?


Personal data we collect is used to provide you with our services and effective clinical care. Your data will also be used for communication between us including confirmation of appointments, sending exercise or treatment plans and other communication about our services. We may need to contact your GP or consultant about your health for example to request further investigations or onwards referral.

How do you store and protect data?


Data is stored on a password protected software under your individual record and is only accessible by clinicians at Baslow Physiotherapy. We use electronic computer systems for all clinical information and any hard copies will be scanned into the system and then shredded. 


Do you share personal data with third parties?


We do not share or sell personal data to any third parties.

We may amend this policy from time to time by publishing a new policy on our website. Please check this policy occasionally to ensure you are happy with any changes. We will notify you of any specific changes via email.
